How Much Does a BBL Cost? Real 2026 Prices and Hidden Fees
A BBL usually costs $8,000 to $15,000 all-in in the US, but the surgeon's fee is only half the story. Here's what a real quote includes and the aftercare costs nobody mentions.
Before
After If you’re asking how much does a BBL cost, the honest range in the US is about $8,000 to $15,000 all-in, with big-city specialists charging $18,000 or more. That number covers more than the surgery itself. A Brazilian butt lift is really two procedures in one: liposuction to collect fat from your tummy, flanks or back, then fat grafting to reshape the buttocks. Every extra area and every extra hour on the table shows up on the bill. Here’s how the price breaks down and the costs that usually aren’t in the first quote.
The short answer
| Cost component | Typical range (US) |
|---|---|
| Surgeon’s fee | $5,000 – $10,000 |
| Anesthesia | $1,000 – $2,500 |
| Facility / operating room | $1,000 – $3,000 |
| Pre-op labs and tests | $100 – $400 |
| Compression garments and supplies | $150 – $500 |
| All-in total | $8,000 – $15,000+ |
The American Society of Plastic Surgeons puts the average surgeon’s fee for buttock augmentation with fat grafting at around $5,000, but that’s an average fee only. Anesthesia and the operating room are billed separately, and they scale with how long you’re under. A BBL often takes three to five hours, which is longer than most breast augmentations.
Why quotes vary so much
Two people can walk into the same office and get quotes $5,000 apart. The usual reasons:
- How many areas get liposuctioned. Many surgeons include two or three areas in the base price and charge $1,000 to $2,500 for each extra one.
- Your body type. If you’re slim, the surgeon may need to treat more areas to collect enough fat. That’s why a “skinny BBL” can end up costing more than a standard one, even though the final change is smaller.
- Location. Miami is the BBL capital of the US, and you’ll find both the cheapest and the most expensive surgeons there. Coastal cities generally run higher than the Midwest or South.
- Surgeon experience. Someone who does BBLs every week and uses ultrasound guidance usually charges more, and that’s often money well spent (more on that below).
- Add-ons. Pairing a BBL with a tummy tuck or a breast augmentation in one trip under anesthesia can save on facility fees, but it pushes the total past $20,000 quickly. Our breast augmentation cost guide has the numbers for that half.
The aftercare costs nobody mentions
This is where budgets fall apart. The quote covers surgery day. The weeks after are mostly on you.
You can’t sit directly on your buttocks for about two to three weeks, and many surgeons want you careful for up to eight. That means a BBL pillow or foam wedge ($30 to $80) for the car and your desk. You’ll wear a stage 1 compression garment right after surgery and switch to a stage 2 a few weeks later, and many people buy two of each so one can be washed. Foam boards and lipo pads add a bit more.
Then there’s lymphatic drainage massage, which a lot of surgeons recommend to help with swelling and lumps. Sessions usually cost $75 to $150, and plans of 6 to 10 sessions are common. That alone can be $500 to $1,500.
Time off is the big one. Most people need about two weeks away from work, longer if your job involves sitting all day or heavy lifting. If you don’t have paid leave, that’s a real cost.
A rough, realistic add-on for aftercare and supplies lands somewhere between $700 and $2,500, not counting lost wages.
Not all the fat stays
Here’s something that affects your long-term cost: only part of the transferred fat survives. Surgeons often say 60 to 80 percent, and it varies a lot from person to person. Most of the shrinkage happens in the first three months as swelling goes down and some fat cells don’t take.
Some people are happy with what’s left. Others go back for a second round, which can cost nearly as much as the first. Ask your surgeon up front what they charge for a touch-up and whether they ever discount it.
BBL prices abroad
| Country | Typical package price |
|---|---|
| United States | $8,000 – $15,000+ |
| Mexico | $3,500 – $6,500 |
| Colombia | $3,000 – $6,000 |
| Turkey | $2,500 – $5,500 |
Overseas packages often bundle the hotel, transfers and garments, which makes them look like a steal. Add flights for you and a companion, a stay of 10 to 14 days (flying too soon raises the risk of blood clots), and the gap narrows. The bigger issue is what happens if you get an infection or a fluid collection after you’re home. Many US surgeons won’t take on complications from someone else’s surgery, or they’ll charge full rates to do it.
Why the cheapest BBL is a risky bargain
A BBL has historically had one of the higher death rates in cosmetic surgery. The main danger is fat being injected into or under the gluteal muscle, where it can enter large veins and travel to the lungs (a fat embolism). In the last several years, professional guidelines have pushed surgeons to inject only into the fat layer above the muscle, and many now use real-time ultrasound to confirm where the cannula is. Safety has improved as a result, but technique still varies from surgeon to surgeon.
So when you compare prices, also compare answers. Ask whether the surgeon is board-certified in plastic surgery, whether they inject subcutaneously only, whether they use ultrasound, how many BBLs they do a year, and whether the surgery happens in an accredited facility. A low price from a high-volume clinic that books several patients a day per surgeon deserves extra questions. This post is general information, so go over your own health history and risks with a qualified surgeon.
Is it covered by insurance?
No. A BBL is purely cosmetic, so insurance won’t pay for any part of it. Most practices offer financing through medical credit companies, but read the terms carefully, since deferred-interest plans can charge you back interest on the full amount if you don’t pay it off in time.
See the shape before you pay for it
Thousands of dollars is a lot to spend on a shape you’ve only pictured in your head, especially when “fuller” means something different to everyone. Pody’s AI plastic surgery simulator lets you upload a photo and preview a subtle or a more dramatic result on your own body. Bringing that image to a consultation makes it much easier to tell a surgeon what you want and to hear whether it’s realistic for the fat you have. If you’re weighing more than one procedure, our breast augmentation recovery timeline shows what combining surgeries could mean for your time off.
